LTC Conference Presentation

His presentation "Using the PAM Survey to deliver: the right support at the right time in the right place" is available here to view or download.

Our approach is not a "One-size-fits-all"

If you haven't already heard, the NHS is under extreme pressure to do more with less money.

Supporting patients with long-term health conditions to self-manage their health can improve clinical outcomes and can lead to more cost-effective use of health services.

Unfortunately, demands on the system ultimately mean that the approach taken to provide self-management is not as patient-centred as patients need.

Self-Care Week 14-20 November, 2016

Self Care Week is an annual national awareness week that focuses on establishing support for self-care across communities, families and generations.

More needs to be done to support people to better look after their own health and empowering individuals to self-care has many benefits for their short-term and long-term health. Helping people to look after their own health, and their family’s health also helps to manage demand on health services.

Last week, a report was published entitled "Supporting self-management; A guide to enabling behaviour change for health and wellbeing using person and community centred approaches". The guide is part of the NHS England-funded Realising the Value programme led by Nesta and the Health Foundation and it discusses the benefits of many of the self-management techniques we teach.
