Commissioning for Value – self-management works

The Commissioning for Value programme for Clinical Commissioning Groups highlights the importance of self-management. The programme, a collaboration between NHS Right Care, NHS England and Public Health England, is running a series of events for commissioners this month. In his presentation, Dr Martin McShane, Director for Long-term Conditions, NHS England, describes two scenarios for the journey of a 45-year old patient diagnosed with diabetes. The first scenario, with no self-management support, costs the NHS £49,000. The second one, in which the patient receives self-management support and has a collaborative care plan, costs the NHS £9,000 - a saving of £40,000 and better health outcomes for the patient.
